Notice, Consents, Declaration and Signature of Applicant
I/We consent to Flato Developments Inc. (“Flato”) collecting and possessing my/our name(s), photographs(s) and "personal information" (as such term is defined in the Personal Information Protection and Electronic Documents Act 2000, c.5) obtained by Flato pursuant to and in connection with this Application Form. I/We acknowledge and agree that the aforesaid information has been provided to Flato with my/our knowledge and consent. In addition, I/we consent to Flato using, releasing, disclosing and/or retaining on file my/our name(s), photograph(s) and personal information, without compensation, to: (a) a company or organization affiliated, associated or related to Flato, in order to provide me/us with information relating to this scholarship program and other scholarship programs of such entities; and (b) any company or third-party entity for the purpose of marketing, advertising and promoting the scholarship program.
I release Flato, its advertising agencies, or other third parties, from all liability in connection with the use and display of my photograph and agree to indemnify and hold them harmless from any claims.
Applicant’s Declaration
1. I have given complete and true information on this application form and in the required supporting documentation.
2. I understand that I am responsible for providing all required supporting documentation as indicated on my application or as directed by FLATO Development Inc in respect to my eligibility for the FLATO Scholarship.
3. I will provide and any change resulting from verification and audit will result in reassessment.
I have read and understood this section, including the notice of collection, use, and disclosure of my personal information and my signature attests to my consent to the indirect collection, use and disclosure of my personal information and that my declaration is complete and true. I understand that any fraudulent or misleading statement may result in revoking an awarded scholarship and the awarded funds shall be returned to FLATO Development Inc.
